SQL Server Database Mirroring immediately switches to Suspended state with no other obvious error

While setting up database mirroring, the mirrored state switches to Suspended immediately after enabling database mirroring. When checking the database mirroring log history errors indicate that SQL Server was unable to connect to the remote server. Resuming mirroring does not produce an error, but immediately after the state returns to Suspended.

We have seen this issue when attempting to establish database mirroring between different service pack levels in SQL Server. Specifically, between SQL Server 2008 R2 RTM and SQL Server 2008 Service Pack 2. The resolution was to bring both instances up to the same service pack version.
